Where does The Nicoyan Diet come from?
The Nicoyan plate is maize treated with lime made into tortillas, black beans and rice as gallo pinto, squash and tropical fruit, fresh cuajada cheese, modest meat, and a great deal of coffee; the region’s hard well water is unusually high in calcium and magnesium. It shares the maize-beans-squash base with the Milpa system, but where Milpa describes a way of farming, this is a specific rural Costa Rican foodway that was measured cohort by cohort in national mortality records — including the finding that its advantage is now fading.

Is there good evidence that The Nicoyan Diet works?
Nidari rates the evidence behind The Nicoyan Diet’s primary claims as mixed — that is the current weight of peer-reviewed research for those claims, and nothing more.
A 2013 study following 16,300 older Costa Ricans found Nicoyan men had a death-rate ratio of 0.80, driven by lower cardiovascular mortality; the advantage did not appear in women and disappeared in out-migrants. A 2024 cohort of 2,827 people linked a traditional rural Costa Rican pattern to 18% lower all-cause mortality. But Rosero-Bixby’s own 2023 analysis shows the advantage vanishing: men born in 1905 had 33% lower adult mortality than other Costa Ricans, while men born in 1945 had 10% higher.
