Where does Exclusive Enteral Nutrition come from?
Surgeons in 1973 noticed that patients put on elemental formula before an operation often had their Crohn’s disease quieten down as well, and asked whether the formula itself was the treatment. A controlled trial in the BMJ in 1984 compared elemental diet against prednisolone and found the diet held its own. It is now first-line induction therapy for children with active Crohn’s across Europe.

The 2018 Cochrane review of 13 trials found enteral nutrition as effective as corticosteroids for inducing remission in children, though steroids may be more effective in adults. The 2021 ECCO-ESPGHAN guideline recommends it as first-line paediatric induction, preferred over steroids because it also supports growth.
Is Exclusive Enteral Nutrition safe, and who should avoid it?
A prescribed medical therapy, not a diet to attempt alone. Requires a gastroenterology team and specialist dietitian; some people need a nasogastric tube. Malnourished patients risk refeeding syndrome when feeding restarts. Not appropriate for obstructing or stricturing disease without surgical assessment. Relapse after normal food resumes is common, so a maintenance plan must be in place before the course ends.
