Where does Six-Food Elimination Diet come from?
Eosinophilic oesophagitis is an allergic inflammation of the food pipe that makes swallowing difficult, and by the mid-2000s the only dietary option was a tasteless elemental formula. Kagalwalla’s team asked whether removing just the six food groups behind most childhood food allergy would work nearly as well. Their 2006 study found it did for most children, and the diet became a standard alternative to steroids.

Is there good evidence that Six-Food Elimination Diet works?
Nidari rates the evidence behind Six-Food Elimination Diet’s primary claims as moderate — that is the current weight of peer-reviewed research for those claims, and nothing more.
The original 2006 study found 74% of children improved. A 2023 meta-analysis of 1,762 patients put histologic remission across elimination diets at 53.8%, highest for the six-food version at 61.3%. The 2020 AGA guideline gives only a conditional recommendation — there is no placebo-controlled randomised trial.
Is Six-Food Elimination Diet safe, and who should avoid it?
Diagnosis and monitoring both require endoscopy with biopsies — symptoms alone cannot tell you whether the inflammation has settled, so this cannot be self-managed. Removing six food groups is a real nutritional risk, especially in children, and needs dietitian oversight for calcium, vitamin D, protein and growth. Many specialists now start with a two- or four-food diet instead.
