Where does Total Diet Replacement come from?
Very-low-calorie formula diets have existed since the 1970s, and early commercial versions using poor-quality protein were linked to around 60 deaths before they were reformulated. The modern medical version was tested in DiRECT, a UK primary-care trial: diabetes medicines were withdrawn, all food replaced by formula for three to five months, then food slowly reintroduced with structured support. The aim is not weight loss for its own sake but stripping fat out of the liver and pancreas.

Is there good evidence that Total Diet Replacement works?
Nidari rates the evidence behind Total Diet Replacement’s primary claims as strong — that is the current weight of peer-reviewed research for those claims, and nothing more.
DiRECT (Lean et al., Lancet 2018) was a cluster-randomised trial in 306 people: 46% of the intervention group achieved remission at 12 months against 4% of controls. The 5-year extension showed 6.1 kg kept off and 36% against 17% at target HbA1c off medication — but of those in remission at year 2, only 26% still were at year 5.
Is Total Diet Replacement safe, and who should avoid it?
Do not attempt without medical supervision. Diabetes and blood-pressure medicines must be reduced or stopped before starting, or you risk dangerous hypoglycaemia. Rapid loss roughly triples the risk of symptomatic gallstones needing surgery. Refeeding syndrome is a risk when normal eating resumes. Contraindicated in pregnancy, breastfeeding, under-18s, and any current or past eating disorder.
